The primary purpose of this position is to assist the Director of Community Relations in the admissions and intake processes of new patients.
The ideal candidate will have experience and the skill set needed to evaluate, coordinate, and admit an average of 40 patients per month.
Requirements:
Supervisory and/or administrative experience in skilled nursing and long term setting is strongly preferred.
Must possess patience, flexibility, tact, a cheerful disposition, and enthusiasm to work cohesively with Interdisciplinary Team, Residents, Family Members, visitors, Insurance Companies, and the General Public.
Knowledge of the Medicare Patient Driven Payment Model (PDPM) and/or experience facilitating Managed Care Reimbursement through negotiated agreements and carve outs is a plus.
Job Duties:
Assist the Director of Community Relations in conducting outreach and education on facility services and availability of patient care services to professional contacts, such as physicians, hospitals, related medical professionals, and discharge planners.
Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed and necessary for patient pre and post admission needs.
Record and report accurate and up to date referral tracker and list of open facility beds on a daily basis.
Collect and Maintain Pre and Post Admission Documents including Benefit Verification Documents, Managed Care Pre-Authorizations, New Admission Paperwork, etc.
Assist in arranging transportation to and from other facilities when necessary.
Participate in touring and orientation of potential residents/family members.
Welcoming and facilitating new admits into the skilled nursing setting, with education and follow up as necessary.
Visitation on routine basis with new admits, assisting in the facilitation of necessary services and coordination with all patient care departments as needed.
Assist family and resident as necessary in the execution of the required admissions documents.
