Schedule :
This position is a part of the Nursing Management Team and will regularly work Monday through Friday with rotating on-call and Manager on Duty.
This position must be willing to work the floor as needed as well.

Role:
We are seeking a dedicated Nurse Case Manager to join our team! The Nurse Case Manager makes assessment for, plans and advocates for the residents of the Communityso they can reach their highest practical level of physical, mental and psychosocial well-being.
The Nurse Case Manager will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community.
Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.
Duties and
Responsibilities:
Duties are determined by the Community's needs and may include, but are not limited to, the following:
Follows and upholds the Community's philosophy and goal to maintain, improve and/or enhance each resident's quality of care and quality of life Perform pre-assessment of residents as necessary.
Attends and participates in facility meetings and in-service training programs as assigned.
Participates in the nurse on-call rotation Promotes positive public relations with residents, family members, guests, and outside referral sources.
Collects and assesses data relevant to resident's psychosocial needs, risk factors for psychosocial deterioration and responses to interventions.
Completes assigned portions of the MDS.
Maintains referral tracker.
Coordinates admissions and discharge planning Reports monthly discharges to the Ombudsman.
Coordinates and directs the care plan meetings.
Facilitates advanced directive decision-making process Coordinates grievance complaint process Coordinate and assist with resourcing and scheduling of allied health services i.
e.
Dentists, Podiatrists, Medical specialists etc Perform all other duties as assigned by the Director of Nursing or Executive Director Qualifications
Qualifications:
Must be a graduate from an accredited school, be currently licensed RN or LPN in state of which applying without encumbrance, and be responsible for maintaining professional licensure and CEU requirements.
Prefer 2 year experience working in a geriatric setting.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
